By attending LIFE Adelaide Playgroup program, I understand that all information contained in this form is confidential. I give permission for such information to be used for statistical and planning purposes. I understand that every caution will be taken to ensure my/my child/ren's welfare while attending this program. I release LIFE Adelaide, and Playgroup from any and all liability in the event of an accident or misfortune, damage or loss that may occur to myself and/or my child/ren and/or my/their property. I give permission for first aid staff to ensure proper treatment for myself and/or my child/ren in the case of emergency and in such case I will pay all doctor, ambulance and hospital fees incurred on behalf of myself and/or my child/ren.
